Department of Surgery Grant Application

All applications with deadlines of April 15, 2015 or later
All applications with DoS faculty in PI, Co-PI or Project/Core leader roles

1) Four weeks prior to the UMN Sponsored Projects deadline (3 months for complex awards, i.e. Multi-institutional; multiple subawards; U,T and K kiosk) faculty to complete online Intent to Submit Form.

2) The faculty member will be contacted by Pre-award staff to arrange a planning meeting.  The meeting aims are to:

  • Ensure that faculty have the support staff needed for the application.
  • Provide up to the minute regulatory and submission guidelines.
  • Discuss the final budget.

Letters of Support

The Chairman’s Office will assist with preparing Letters of Support on behalf of the Department of Surgery and/or Dr. Ikramuddin. Please submit a draft version of the Letter of Support (Word doc) , current CV and an abstract or summary of the associated application to Emily Kowalczyk in the Chairman’s office.

Submitters should allow five business days for the processing of Letters of Support.

Proposal Routing Forms

The signing chain for all Proposal Routing Forms (PRF) is Department Head/Delegate: Lisa Rogers (roger031) and Department Administrator: Randa Rosby (randa) or Sanoa Hagen (s-hage). 
Drs. Chipman (chipm001), Beilman (beilm001) and Ikramuddin (ikram001) serve as alternate signers when Lisa Rogers is unavailable. Please contact the Chairman’s office to verify that an alternate signer is needed.

Submitters should allow three business days for the processing of PRF signatures.

F & A Waiver Forms

F & A waivers are granted only for business reasons. F & A waivers must be approved prior to submission of PRFs. The rationale for F & A waiver must be clearly stated. The Department Chair, Sayeed Ikramuddin is the signatory for all F & A waivers. It is recommended that F & A waiver requests be discussed with Dr. Ikramuddin prior to submission of the forms.

Submitters should allow five business days for the processing of F & A waivers. F & A waiver forms should be submitted to the Chairman’s Office with a copy emailed to Lisa Rogers

No Cost Extension Forms

The Department Chair, Sayeed Ikramuddin is the signatory for all letters of request for No Cost Extensions.

Submitters should allow three business days for the processing of letters. Letters should be submitted to the Chairman’s Office with a copy emailed to Lisa Rogers

PreAward and Advance Accounts

PreAward and Advance accounts obligate the Department of Surgery to fulfill all financial obligations if the anticipated funding is not received. For this reason, all requests must include a non-sponsored chart string that can be used should the anticipated funding not be received. The Department Chair, Sayeed Ikramuddin is the signatory for all PreAward and Advance accounts.

Submitters should allow three business days for the processing of PreAward and Advance Accounts. Requests should be submitted to the Chairman’s Office with a copy emailed to Lisa Rogers

Research Funding Gaps

Researchers should contact the Chairman’s Office as soon as possible to discuss anticipated gaps in research funding.

Fostering Integrity in Research, Service, and Teaching (FIRST) provides training for Investigators including Responsible Conduct of Research (RCR) required initial training and continuing education.

The University of Minnesota requires a basic level of Human Subjects' Protection Training for all investigators and research personnel conducting researching involving human subjects, regardless of the source of funding.

IACUC

IACUC

In accordance with federal, state, and institutional regulations and policies, all personnel at the University conducting research involving animals must complete IACUC training.

Fairview

Fairview

Fairview-employed Investigators, unaffiliated Investigators and key personnel conducting research with Fairview patients, records, specimens, systems & resources who are involved with recruiting, consenting or data collection/abstraction must complete the required Investigator Education training. Training needs to be completed once every three years.
